Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's responsibility to notify tenants of lead related reports and tests, and to ensure that this unit complies fully with 105 CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an inspection, contact the Salem Board of Health at 978-741-1800. You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection report and to take all positive action to prevent these violations from occurring again in the future. Violation(s) Based upon a tenant complaint, an inspection was conducted in accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon inspection, the following were noted: 7 days .351 1. Bathroom sink has a leak underneath it and the inside of the cabinet has mold/mildew inside it, likely as a result of the leak. Hire services of licensed plumber to repair the leak so it does not reoccur: dry out the cabinet, clean mildew/mold under sink and repair/repaint so the cabinet is intact and in good condition. Owner to correct this violation within 7 days. 7 days .253(A) 2. There is no exterior light fixture at the front entry to the building. Owner must provide light fixtures in good working order so that illumination can be provided for the safe and reasonable use of every porch and exterior stairway; install a light fixture at the front entry to the building. Owner to correct this violation within 7 days. 24-48 hours .550 3. Some mouse droppings were noted in a corner of the living room under the baseboard heaters. Owner to hire services of a licensed exterminator to inspect and treat the building for mice;treatments must continue until problem is abated. Bottom sash on the window in the common closet on the third floor does not stay in place or line up to lock with top sash. Window must be repaired/replace so it is intact and in good working order.
