The Board of Health has received notification that the property located at 83 Tremont Street in Salem,
Massachusetts is in violation of one or more of the following Board of Health regulations as indicated:
BOB Regulation 7,Sec 5.4:More than one bulk waste item placed at curbside.
The violation specifics are as follows:
Mattress and box spring left at curbside on Tremont Street in front of your property.
You are ordered to correct the above violation(s)within twenty-four(24)hours.
Should you be aggrieved by this order,you have the right to request a hearing before the Board of Health.A request
for said hearing must be received in writing in the office of the Board of Health within 7 days of receipt of this
order.At said hearing,you will be given an opportunity to be heard and to present witness and documentary
evidence as to why this order should be modified or withdrawn. You may be represented by an attorney.Please also
be informed that you have the right to inspect and obtain copies of all relevant inspection or investigation reports,
orders,and other documentary information in the possession of this Board,and that any adverse party has the right
to be present at the hearing.
Continuation of this violation or its recurrence will result in the issuance of a$25 citation for each day the violation
exists.
