State Sanitary Code,Chapter 1:General Administrative Procedures and 105 CMR 410.000:State Sanitary Code,
Chapter 11: Minimum Standards of Fitness for Human Habitation,an inspection was conducted of the property 11
Summer Street Unit 1 R permitted by occupants Paul Morin&Linda Morin conducted by Jeffrey Barosy,
Sanitarian on February 9,2023 at 2:00pm.
Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's
responsibility to notify tenants of lead related reports and tests, and to ensure that this unit complies fully with 105
CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an
inspection,contact the Salem Board of Health at 978-741-1800.
You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection
report and to take all positive action to prevent these violations from occurring again in the future.
Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint
being sought against you in Trial Court. Time for compliance begins with receipt of this Order.
Should you be aggrieved by this Order,you have the right to request a hearing before the Board of Health. A request
for said hearing must be received in writing in the office of the Board of Health within 7 days of receipt of this Order.
At said hearing,you will be given an opportunity to be heard and to present witness and documentary evidence as to
why this Order should be modified or withdrawn.You may be represented by an attorney. Please also be informed
that you have the right to inspect and obtain copies of all relevant inspection in investigation reports,orders and other
documentary information in the possession of this Board,and that any adverse party has the right to be present at the
hearing.

Based upon a tenant complaint a permitted inspection was conducted in
accordance with Article II of the State Sanitary Code; 105 CMR 410.000. Upon
Inspection the following were noted:
.500 1. Bathroom ceiling tiles show signs of water damage with ceiling tiles
24 Hours with discoloration and peeling surfaces. Find source of water leak
and repair. Replace effected ceiling tiles. Owner has 24 HOURS to
start making agood-faith effort to correct violation.
.500 2. Bathroom walls behind sink and toilet are deteriorating with chipping
7 Days and peeling paint and exposed brick. Repair bathroom walls so they
have smooth and easily cleanable surfaces. Owner has 7 DAYS to
start making agood-faith effort to correct violation.
.500 3. Bathroom floor tiles are cracked and warped and there is a hole in
7 Days the floor behind the toilet. Repair bathroom floor so it is free of holes
and other defects. Owner has 7 DAYS to start making a good-faith
effort to correct violation.
.500 4. Kitchen floor near entrance to bathroom has a hole. The floor below
7 Days the dwelling unit is visible through this hole. Repair hole in kitchen
floor. Owner has 7 DAYS to start making a good-faith effort to correct
violation. _
.500 5. Kitchen window on right has a broken string for the sash's
7 Days .480 counterbalance on the right side. The same window lacks a catch for
the lock. Repair this window's sash's counterbalance string. Provide
a catch for the lock so window can be locked properly. Owner has 7

.500 6. Kitchen window on the left has sashes that do not align, preventing
7 Days window from being locked. Repair window so sashes are properly
aligned and can lock properly. Owner has 7 DAYS to start making a
good-faith effort to correct violation.
.500 7. Bedroom adjacent to kitchen has a hole in the floor around a vertical
7 Days pipe in one of the corners of the room. Provide an escutcheon for this
pipe to cover the hole in the floor. Owner has 7 DAYS to start making
a good-faith effort to correct violation.
.351 8. Bedroom adjacent to kitchen has a ceiling light that does not stay on
7 Days when activated. Repair ceiling light so it turns on and off with ease.
Owner has 7 DAYS to start making a good-faith effort to correct
violation.
.500 9. Pantry window's sashes do not align, preventing window from being
7 Days locked. Repair window so sashes are properly aligned and can lock
properly. Owner has 7 DAYS to start making a good-faith effort to
correct violation.
.500 10. Pantry floor in front of sink is warped and deteriorated. Repair pantry
7 Days floor. Owner has 7 DAYS to start making a good-faith effort to correct
violation.
.550 11. Pantry cabinets near floor across from sink have mice droppings.
24 Hours .750 (0) Owner is to exterminate mice in the dwelling unit and all adjacent
walls. Any holes leading into the dwelling unit must be plugged to
prevent re-entry from mice. Extermination is to continue as
needed until mice no longer present a public health nuisance in
the dwelling unit. Owner has 24 HOURS to start making good-faith
effort to correct violation.
.351 12. Interior floor of cabinet underneath pantry sink has rotted away.
7 Days Provide an intact interior floor for cabinet under pantry sink. Owner
has 7 DAYS to start making agood-faith effort to correct violation.
.500 13. Living room ceiling has chipping and peeling paint above ceiling light.
7 Days Remove chipping and peeling paint and repaint effected areas.
Owner has 7 DAYS to start making a good-faith effort to correct
violation.
.500 14. Living room's windows' sashes do not stay open under their own
7 Days .501 power, needing to be propped open. Repair living room windows so
their sashes open and close with ease and can stay open under their
own power. Owner has 7 DAYS to start making a good-faith effort to
correct violation.
.500 15. Living room's middle window's top sash is screwed shut. Remove
7 Days .501 screws and repair sash so it opens and closes with ease and can
stay up under its own power. Owner has 7 DAYS to start making a
good-faith effort to correct violation.
.500 16. Living room floor has a hole in the area between the windows and the
7 Days adjacent bedroom's door. Repair hole in living room floor. Owner has
7 DAYS to start making agood-faith effort to correct violation.
.500 17. Hallway in front of living room has a portion of its ceiling plaster fallen
7 Days away. Repair this area of hallway's ceiling. Owner has 7 DAYS to
start making a good-faith effort to correct violation.
.500 18. In the hallway in front of the living room there is a hole in the floor
7 Days around a vertical pipe next to the entryway into the living room.
Provide an escutcheon to cover the hole in the floor around pipe.
Owner has 7 DAYS to start making a good-faith effort to correct
violation.
.500 19. Bedroom adjacent to living room has evidence of water leakage with
24 Hours brown discoloration of plaster above entryway into living room. Find
source of water leak and repair. Remove discolored portion of plaster
and replace with new plaster. Owner has 24 HOURS to start making
a good-faith effort to correct violation.
.482 20. Bedroom adjacent to living room does not have a carbon monoxide
24 Hours .750 (N) detector. Provide a carbon monoxide detector and have it placed
within 10 feet of this bedroom's entrance. Owner has 24 HOURS to
start making a good-faith effort to correct violation.
.500 21. Bedroom adjacent to living room has a single stand-alone window
7 Days with sashes that do not align, preventing window from being locked.
Repair window so sashes are aligned and can lock properly. Owner
has 7 DAYS to start makin a ood-faith effort to correct violation.
.500 22. Bedroom adjacent to living room has a group of windows across from
7 Days .501 entryway into living room. The middle window has a bottom sash that
does not stay open under its own power. Repair this window's sash
so it can open and close with ease and stay open under its own
power. Owner has 7 DAYS to start making a good-faith effort to
correct violation.
.500 23. Bedroom window adjacent to living room has a hole in the floor in the
7 Days corner between the entryway to the living room and the stand-alone
window. Repair the hole in the floor. Owner has 7 DAYS to start
making agood-faith effort to correct violation.
N/A 24. NOTE: Occupants informed inspector that an outlet in the bedroom
N/A adjacent to the living room on the wall across from the stand-alone
window did not work. The inspector could not confirm whether this
outlet worked at the time of inspection because the inspector's three-
pronged outlet tester could not fit in the ungrounded outlet. Inspector
advises owner to have this outlet inspected by a licensed electrician
and repaired if it is not working ro erl .
N/A 25. Owner has 30 DAYS to correct all violations listed in this report.
