This binder will include the standard protocol for pest issues,logs to report pest
complaints/sightings, reports of services rendered during pest treatments,and
Safety data sheets for all products used on the premise
b. IP `ervices.deco mendations:
i. Based on the location of the buildings and the pest history,the Pest Management
Professionals recommend a monthly pest suppression program.The monthly
pest program will include inspection/treatment for standard pest control(see
list of standard pests on the following page)in the following areas: foyer of each
building,the leasing/management office, all utility rooms, electrical rooms,
elevator rooms, compactor rooms,laundry rooms,trash rooms, community
rooms,basements and exterior perimeters of the buildings.
1. Additional services included with contracted accounts:
a. all units treated for standard pests that are deemed"moderate to
heavy"levels of activity will receive a complimentary follow-up
inspection/treatment within 14 days of the initial treatment.
b. IMPORTANT REMINDER: Exclusion services(blocking of small
rodent entry holes)is included with service in units and common
areas.
c. Included with the pest service are the following reports:
i. a site inspection report on all units/areas detailing the
type/level/location of activity found,the sanitation level,
and any recommendations to minimize future pest issues
ii. A graph indicating the location of each exterior station and
the level of activity found
iii. Photographs of any structural or sanitation issue conducive
to pest activity found throughout the tenant units

application (consisting of a contact kill&a residual product) to all beds,
frames, nightstands, wooden furniture, upholstered furniture, base boards,
door frames&rugs if applicable.
2. 2ndvisit:A liquid application to all beds,frames, nightstands, wooden
furniture, upholstered furniture,base boards, doorframes&rugs if
applicable.A HEPA vacuum treatment to all furniture in order to knock
down the bed bug population in all stages(especially nymph and egg)and a
insecticide dust application to the wall voids throughout the unit.
3. 3rd visit:An inspection of the tenant unit to assess the level of visible activity
found at the current time and a spot treat of all furniture as needed This
will be determined upon the findings during the inspection.At the
completion of treatment, the service technician will install monitors to
access any potential future bed bug activity.

PMPs will date, check,and replace all monitoring devices are necessary.
12.We only use EPA registered products&can provide Safety Data Sheets (SDS) and labels of
all products in question.

1. Timely Payment of all work performed

a. After 90 days of delinquency customer accounts will be put on hold until payment is
received
b. Once an account goes into delinquency the customer must put a credit card on file in
order to activate account.
i. This credit card will be used only when account balance exceeds 90day
status.
3. Automatic Payments (eligible for 5% discount)
I authorize Elite Pest Solutions to automatically debit my bank account or credit card, as
indicated below, in an amount equal to any recurring service charges due under this
agreement within seven (7) days of the date such charges become due.This authorization will
remain in effect until the seventh day following the service receipt or upon receiving a written
notice to cancel such authorization.

1. The term of this agreement shall be a period of one (1)year beginning on the date
executed.Thereafter,this agreement shall automatically renew for additional one (1) year
periods unless terminated in accordance with this agreement.
a. Either party may terminate this agreement by providing at least 30 days advanced
written notice
2. Interim service visits, at the request of the customer,will be subject to a service fee of
$75.00&UP depending on the severity of the pest situation.
3. Customer must allow properly identified pest management professionals access to the
structures for any purpose contemplated by this agreement.
4. Customer cooperation is vital to ensure the most effective solutions from service.Any
conditions conducive to the breeding and harborage of pests covered by this contract are
documented onsite inspection reports by the PMP to the customer,and are not corrected
by the customer, Elite Pest Solutions cannot ensure effective services. If customer fails to
correct the noted conditions within a reasonable period,any guarantee of the effectiveness
of service will be terminated.
S. Upon transfer of ownership of the structure and/or the business services may be continued
upon request of the new owner. In addition, Elite Pest Solutions reserves the right to revise
pricing upon transfer of ownership.
6. In case of non-payment or default, Elite Pest Solutions has the right to terminate this
agreement. In addition, cost of collection including attorney fees,shall be paid for by the
customer.
7. At the time of renewal both the customer and Elite Pest Solutions may change the
agreement by modifying any provision.All parties must be notified in advance of any such
changes and either party may decline to accept such a change by declining to renew this
agreement.
8. This agreement may be modified at any time with the written consent of both parties.
