Dear Sir/Madam:
In accordance with Chapter fl of the State Sanitary Code, 105 CMR 410.000, Minimum Standards of
Fitness for Human Habitation, a re-inspection of your property located at 98 North Street#21-was
conducted by Elizabeth Gagakis, Senior Sanitarian of the Salem Board of Health on February 20, 2018.
The following violations from the report dated January 3, 2018 are still outstanding:
�3. Electrical cross wiring— no documentation has been provided at this time indicating that a house
meter has been successfully installed and connected and that all appropriate fixtures and wiring have been
connected to it. At the time of inspection it was observed that tenant's kitchen light and light in back hall on
second floor are on the same switch, controlled by a breaker in the back hall on the second floor. Electrical
work must continue and be appropriately resolved, and an invoice must be provided from the licensed
electrician doing the work with written details on the work that was performed,
7. Tub wall is worn and in disrepair some patching has been done but this area is still discolored and
in disrepair.
15. Glass Is separating from the bottom sash in left living room window—some sealing has been
done but glass can still be pushed out of the sash.
18. No working light fixture in back hallway on the first floor— no permanent light fixture has been
installed.

Based upon a tenant complaint a permitted inspection was conducted in accordance with
Article II of the State Sanitary.Code, 105 CMR 410.000. upon Inspection the following
were noted:
7 days .500 1. Left living room window does not lock. Repair so sashes align properly and window
is able to be locked. Owner to correct this violation within 7 days.
7 days .551 2. Right living room window has a damaged screen. Repair screen so it is intact and
in good condition; screens must be installed between April 1 and October 30, both
inclusive,each year. Owner to correct this violation within 7 days.
7 days .500 3_ One kitchen window does not stay up when opened. Repair window so it works
properly and is able to be opened. Owner to correct this violation within 7 days.
7 days .480 4. Lock on the back entry door is not operable. Repair so door is able to be locked
properly. Owner to correct this violation within 7 days.
7 days .500 5. The doorjamb at the back entry door is damaged. Repair so doorjamb is in good
condition.Owner to correct this violation within 7 days.

7 days .500 6. Window in son's beddroom does not stay up when opened. Repair window so it
works properly and is able to be opened.Owner to correct this violation within 7
days.
14 days .500 7. There is what appears to be water damage in the back hall at the duct, evidenced
by cracks/damage and staining on the wall/ceiling. Investigate for source of any
leaks and repair;repair walls/ceilings/any damaged areas in this hallway so they
are Intact and free from leaks and damage. Owner to correct this violation within 14
days.
7 days .500 8. There is a hole inside the living room closet in the ceiling/wall at the duct. Repair
hole so ceiling/waif are intact. Owner to correct this violation within 7 days.
24-48 hours .482 9. There did not appear to be a carbon monoxide detector in the unit at the time of
inspection. CQ detectors must be placed within ten feet of every sleeping area.
Owner to correct this violation within 24-48 hours; referred to Fire Prevention for
follow-up.
24-48 hours .482 10. Smoke detector in the master bedroom was missing from bracket.Smoke detector
must be in place and in working condition at all times. Owner to correct this
violation with-74=48--hours; roferred 16 Fire Prevention for foilaw-up.
