2. Plumbing certification stated all water, sewer and gas service enters/leaves through
Edgewater cafe. Where are services in the other buildings, how do they connect to the pipe
leaving/entering Edgewater cafe?
3. Plumbing certification references permits pulled for capping A/C drain lines, what lines

4. Demo regulation requires engineered stamped plan, if utilities are to be re-used
immediately and not capped at the city's main. Provide engineered stamp drawing

Agreement Memo of Conditions to Approve Demolition Plan
This memo has been prepared to summarize the conditions of approval agreed upon by RCG and
the City of Salem Engineering Department. The writer met at 9:15 AM on October 31, 2007 with
the above referenced representatives of RCG in the Engineering Department to review the
Engineering Department's comments on the plan that are summarized in the attached memo dated
the same day.
It is understood by the City that due to the age and type of construction of the buildings, it has
been impossible for the owner to ascertain the location and condition of all water, sewer and drain
lines leaving and/or entering the buildings to be demolished. Therefore, that information is
incomplete on the plan submitted.
All comments were reviewed and the following was agreed by all:
1. Incomplete information will be provided n a revised plan, stamped by a professional
engineer, who completes a survey of piping entering and leaving the buildings in a
reasonably short period of time after the buildings have been removed. RCG agreed to
require their demolition contractor to take care not to bury or cover any pipes during his
work and to overall assist in the process of providing an accurate plan of utilities. This
agreement satisfies a majority of the comments as follows: overall comments: 2, 3, 4;
sewer: 2, 3; and water: 2, 3.
2. As part of the survey by a professional engineer, the existing sewer in the Washington
Street Mall and drainage system in the same mall and other locations on the property

will be cleaned and inspected to ascertain condition and connectivity. Any defects
found will be repaired by the owner. This agreement satisfies the following comments:
overall comments: 1; sewer 1; and drainage: 1, 2, 3, 5.
3. RCG stated the certification by Keyspan is accurate. Keyspan told them there is only
one service into the buildings through the Edgewater cafe. This satisfies the following
comment: overall comment: 5.
4. RCG stated the City had shutoff the water to #26 Front Street years ago. The writer
will confirm with DPW. This satisfies the following comment: water comment: 4.
5. RCG stated all roof drains spill to the ground and are not connected to anything
underground. This will be confirmed with the engineer survey following demolition.
This satisfies the following comment: drainage: 4.
6. RCG stated the Salem Fire Department required the fire service to be capped today, as
close as possible to the day of demolition. This satisfies the following comment: water:
1.
With all the comments in the City's attached memo addressed and agreed upon herein. The City
engineering Department approves the Demolition Plan for water and sewer.

Dust suppression,control from this demolition operation is a high priority for this Salem
project. Wetting with hand held hoses shall be used to control, mitigate and maintaina
f reasonable level of dust_During the demolition process a labor will fix a steam Of water
on the demolition impact-and surrounding area.
The controlled piles of concm:e,brick and wood debris will also receive a steam of water
to maintain and control a reasonable level of dust.
Sufficient water shall be applied to meet Federal, State and local air quality regulations.
All trucks will be covered when transporting debris from this demolition zone.
Testa Corp will cooperate with Local agencies to insure and maintain a safe and clean
site.

We completed a full investigation of utilities at 155-189 Washington Street,26 New
Derby Street and 31 Front Street on Monday October 8,2007. Water,sewer and gas
service all enter the building off Front Street under Edgewater Cafd. The water and
sewer connection have been cut and capped excluding services for the remaining
restaurants.
All A/C units refrigeration have been reclaimed and plumbing permits have been sent to
plumbing inspector for capping drain lines. If you need any other information please call
