Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's responsibility to notify tenants of lead related reports and tests,and to ensure that this unit complies fully with 105 CMR 460:000: Regulations for Lead Poisoning Prevention and Control. For further information or to request an inspection, contact the Salem Board of Health at 978-741-1800. You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspection report and to take all positive action to prevent these violations from occurring again in the future. Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint being sought against you in Trial Court. Time for compliance begins with receipt of this Order. Should you be aggrieved by this Order,you have the right to request a hearing before the Board of Health. Violation(s) Based upon a tenant complaint a permitted inspection was conducted in accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon Inspection the following were noted: 24 Hours .351 1. The following areas have wall mounted outlets with either loose fitting .750 (K) covers, outlets coming out of the walls, or both: the bedroom first from the bathroom on the wall adjacent to the second bedroom, the bedroom second from the bathroom on the wall adjacent to the first bedroom, and the living room wall on the area under to the thermostat. Repair these outlets so they are properly affixed inside the wall and their covers are flush to the wall..Owner has 24 HOURS to start making a good-faith effort to correct violation. .351 2. The living room's baseboard heater under the window towards the left 7 Days has a cover that cannot stay on. Repair this baseboard heater and its cover so the cover stays on properly. Owner has 7 DAYS to start making agood-faith effort to correct violations. .500 3. The kitchen ceiling above the stove has cracking plaster. Repair 7 Days effected area of kitchen ceiling so plaster is no longer cracking. Owner has 7 DAYS to start making a good-faith effort to correct violation. .351 4. The kitchen drawer doors directly below the kitchen sink are broken. 7'Days Repair drawer doors. Owner has 7 DAYS to start making a good-faith effort to correct violation. .351 5. The kitchen cupboard beneath the counter on far left next to 7 Days refrigerator has an interior floor that is bowing. Repair the interior floor of this cupboard so it is no longer bowing and is in good repair. Owner has 7 DAYS to start making a good-faith effort to correct violation. .500 6. The kitchen walls plastic baseboard is wet and loose from a previous 24 Hours .750 (K) sewage overflow. Dry out kitchen wall and repair water damaged areas. Owner has 24 HOURS to start making a good-faith effort to correct violation. .500 7. The living room door leading to the HVAC unit shows signs of water 7 Days damage from a previous sewage overflow. Either replace this door or repair damaged portion of door. Owner has 7 DAYS to start making a good-faith effort to correct violation. .500' 8. The hallway wall across from the kitchen that forms a corner with the 7 Days living room wall shakes when walls in first bedroom from bathroom is struck. The City's Building Department was consulted, and the inspector was informed the hallway wall may not be structurally sound. Either provide proof this hallway wall is structurally sound or repair wall so it is structurally sound. Owner has 7 DAYS to start making a good-faith effort to correct violation. _ N/A 9. NOTE: Occupant mentioned seeing roaches in the dwelling unit. At N/A the time of inspection, the inspector did not observe any evidence of roach activity. Inspector advises Owner to have this investigated by licensed exterminators. N/A 10. NOTE: Occupant expressed concern over covered vents in the N/A kitchen and bathroom. Inspector was informed by City's Building Department that covered vents are complying with building code, but new vent in bathroom needs to vent to the outside. Inspector will inquire from property management if this vent vents air to the outside. N/A 11. Owner has 30 DAYS to correct all violations listed in this report. N/A Owner must start making good-faith effort (reporting to the Salem Board of Health what actions will be taken in writing, hiring contractors that may be needed, or starting necessary work on his/her own) within the time frame noted beside each violation and within each violations' description.
