State Sanitary Code,Chapter 1:General Administrative Procedures and 105 CMR 410.000:State Sanitary Code,
Chapter 11: Minimum Standards of Fitness for Human Habitation,an inspection was conducted of the property 41
Hancock Street Unit 2 permitted by occupant Longinus Ehioriodo Prestige conducted by Jeffrey Barosy,
Sanitarian on December 28,2022 at 10:00am.
Notice: if this rental unit is occupied by a child or children under the age of 6 years, it is the property owner's
responsibility to notify tenants of lead related reports and tests,and to ensure that this unit complies fully with 105
CMR 460:000: Regulations for Lead Poisoning Prevention and Control: For further information or to request an
inspection,contact the Salem Board of Health at 978-741-1800.
You are hereby ORDERED to make a good-faith effort to correct the violations listed on the enclosed inspecfion
report and to take all positive action to prevent these violations from occurring again in the future.
Failure on your part to comply within the time specified on the enclosed inspection report will result in a complaint
being sought against you in Trial Court. Time for compliance begins with receipt of this Order.
Should you be aggrieved by this Order,you have the right to request a hearing before the Board of Health. A request
for said hearing must be received in writing in the office of the Board of Health within 7 days of receipt of this Order.
At said hearing,you will be given an opportunity to be heard and to present witness and documentary evidence as to
why this Order should be modified or withdrawn.You may be represented by an attorney. Please also be informed
that you have the right to inspect and obtain copies of all relevant inspection in investigation reports,orders and other
documentary information in the possession of this Board,and that any adverse party has the right to be present at the
hearing.
Please be advised that the conditions noted may enable the occupant(s)to use one or more of the statutory remedies

Based upon a tenant complaint a permitted inspection was conducted in
accordance with Article II of the State Sanitary Code, 105 CMR 410.000. Upon
Inspection the following were noted:
24 Hours .351 1. Outlet in living room on wall adjacent to bathroom does not work.
Have a licensed electrician repair outlet so it provides electricity as
designed. Owner has 24 HOURS to start making a good-faith effort to
correct violation. .
.500 2. At the bottom of the front common stairway, to the right when entering
7 Days from the outside there is a hole in the wall. Repair wall so that it is
intact and free of defects. Owner has 7 DAYS to start making a good-
faith effort to correct violation.
N/A 3. NOTE: The Occupant initially called the Salem Board of Health for a
N/A no-heat complaint. At the time of inspection the heat in the living room
and kitchen was measured at 60*17, five feet from exterior walls and
"five feet above the floor. Two thermostats were observed in the
occupant's dwelling unit with one not working and the other working
and displaying a temperature of 57*F. The inspector requested to go
to the basement to assess the heating system, but the Occupant
informed the inspector that the basement was locked by the Owner.
When asked, the Occupant informed the inspector that the
occupants, not the Owner, were responsible for filling their respective
oil tanks with oil. Later that day the inspector called the Owner and
requested he open the basement so the heating system could be
assessed. The Owner complied and an inspection was scheduled for
the earliest date possible, January 3, 2023. During this phone
conversation the inspector asked who was responsible for filling the
oil tanks and the Owner confirmed that the occupants were
responsible for filling their own oil tanks. On Janua 3, 2023 at
9:OOam the Owner let the inspector into the basement where the
inspector observed that the Occupant's dwelling unit's heating system
turned on and off when its switch was activated, and the same
dwelling unit's oil tank was empty. The inspector has determined a
proper assessment on whether the Occupant's dwelling unit's
heating system works properly cannot be determined until its
respective oil tank is filled with oil. The Occupant will need to fill
the oil tank. The inspector will conduct another inspection after
the oil tank has been filled and the thermostat has been set to at
least 68*F.
N/A 4. Owner has 30 DAYS to correct all violations listed in this report.
N/A Owner must start making good-faith effort(reporting to the Salem
Board of Health what actions will be taken in writing, hiring
contractors that may be needed, or starting necessary work on
his/her own)within the time frame noted beside each violation and
within each violations' description.
One or more of the above violations may endanger or materially impair the health, safety and well-being of the
occupant(s)
