Date of Acceptance: Signature: CONTRACTORS RESPONSIBILITIES This outline is intended to establish minimum quality standards for build out of 38' rear dormer with complete second level renovations, including selective first floor renovations according to these specification sheets on 1 Riverbank Rd., but is not intended as a complete specification for construction purposes. The renovations shall be constructed to the design provided by MDB Construction. It is understood that the contractor will make all necessary provisions to ensure that the renovation will be in compliance with all applicable local state codes and regulations,It is fiirther understood that construction will be completed in a workmanlike manner in keeping with the standards of the industry. I. DEMOLITION • All demolition necessary for the construction renovations in accordance to plans. • Removal of rotted wood at bulkhead location. • Removal of wood fire hazard in first floor chimney. • Remove all wallpaper on first floor walls. • Remove all first floor carpet. • Demolition of existing bulkhead roofing. • Removal of existing boiler. • Provide on site containers if possible, or removal by truck of all job related debris. 2. EXCAVATION/FOUNDATION/SLAB • None 3. PARTITIONS • EXTERIOR: All exterior partitions shall be constructed of 2x4 wood stud, 16"o.c., '/2" plywood sheathing(CDX), Tyvek air infiltration barrier, Wolverine or equal full coverage vinyl siding to match existing as close as possible, 3 '/2", (R-13)wall insulation, '/2 blueboard(skim coat plaster). • INTERIOR: All new interior partitions shall be 2x4 construction, 16"o.c., ''/z"blueboard (plastered). �C� • 4. FLOOR • Wood joist floor framing to be existing 2x10 hem/fir and '/<"sub-floor, Y2 AC,or equal finish plywood attached as necessary. • Finish floor of second level to consist of wall to wall carpet and ceramic tile, as per plans(carpet materials and installation allowance of$1,500.00, ceramic tile materials allowance of$256.00). • Refinishing of existing hardwood floors exposed after carpet demolition with one coat of sealer,and two coats of finish(clear). Note: Any patching will be considered an extra, 5. ROOF/CEILING • Wood frame system to be constructed of 2x10 wood rafters, 2x8 ceiling joists, ''/z" blueboard(skim coat plaster(textured)),R-30 insulation(new only), 5/8"plywood, 15 LB felt paper, standard asphalt roof shingles, ice and water barrier, 1x6 #2 rakes, 1x8#2 facias, 3/8" plywood soffit, ,and ridge vent system. • Installation of 45 mils EPDM rubber roofing over existing bulkhead area. • 6. WINDOWS/DOORS • Installation of four new construction vinyl tilt wash windows to have a materials allowance of$1,200.00. Note: Existing windows to remain. • All new interior doors to be Wlow.core luan pre-hung door units as per plans. 7. PLUMBING • Installation of all plumbing necessary to supply,waste, drain, and vent in accordance to code. One new three-fixture bath. Note: Plumbing fixture allowance of$1,500.00 includes vanities, faucets,tub, shower valve,and sinks). Note: Custom hand held shower units may incur additional charges. • Installation of new 50 gal gas fired hot water heater. • Installation of all gas piping necessary for new boiler, and hot water heater only. • Medicine cabinet, mirrors, and bathroom accessories to be supplied by owner, and installed by MDB. gc(s i 8. HEATING • Installation of new hydronic baseboard heat for second level to match existing as close as possible. • Installation of owner supplied gas fired boiler. Note:Includes installation of existing zones of heat. Note: Includes installation of one additional zone for new second level. 9. ELECTRICAL • Installation of all necessary electrical so as to conform to code. Note: All light fixtures to be supplied by owner. • Installation of one new thermostat(standard contractor line). • Installation of power wiring for new boiler, water heater,and dishwasher. Note:Any recessed lighting, or paddle fans to be considered an extra. 10. SPECIALTY TPEMS • Installation of all necessary wood trim to match existing as close as possible. • Installation of repairs to rotted wood at bulkhead. • Installation of repair to existing broken damper in existing fireplace.-- Note: If replacement is necessary additional charges will apply. • Installation of one coat of primer,and two coats of finish paint over all new work, and at old wallpaper locations. Note: Walls only where wallpaper was removed. Note: Ceilings on new second level to be textured so no paint will be applied. • Installation of fir wall attached handrails as necessary on existing stair unit to new second level. • Installation of seamless aluminurn gutters on new dormer. EXCLUSIONS • Any unforeseen conditions. Note: MDB describes an unforeseen condition as any additional work required due to a condition that was not visible or known by the builder during the walk through or pricing stage. • Repairs to any landscaping,or driveways caused by dumpsters,trucks, excavation, etc. Note: MDB will do their best to protect property,but cannot be held responsible for normal construction damage. • If the homeowner chooses to reside in the home during renovations,MDB Construction will not be responsible for problems due to airborne particulate, hazardous construction situations or homeowner caused delays. It is the intent of the contractor to have a working crew Monday through Friday 7:30 to 4:00. It will be the homeowners responsibility to ensure that the job site is in a work ready environment during our crews working hours. • MDB Construction will not be responsible for moving or protection of homeowner's furniture and personal possessions due to homeowner residing in the home during the renovations or during hardwood floor refinishing. Furniture moving will incur additional cost. • All appliances to be by supplied to the job site by owner, including delivery onto the • floor where the work is being done. • Gas Co. to supply all gas piping from street into home. • MDB Construction to not be responsible for any variances, and or special permits. 12. ADDffIONAL WORK The Contractor shall perform additional work as requested and approved, in writing, by The Owner and the Contractor for a sum specified. The Contractor shall make at any time during the life of this Agreement any revisions, additions,deletions, modifications, corrections, substitutions or changes to the Scope of Services as may be agreed to by both parties to this Agreement for a sum specified. 13. INSURANCE The Contractor warrants that it is fully covered by the types of insurance listed below and that it will maintain said insurance at all time while the Work is being performed. When requested,the Owner shall be provided with Certificates of Insurance. 1. Workers Compensation as required by the law of Massachusetts; 2. Comprehensive General Liability Insurance for a combined single limit of one million dollars ($1,000.000)for bodily injury,death, and property damage; QG� 14.
