On May 1", 2019,I was asked by Salem P.D to look at the condition of your property during a search warrant of
the property. The interior of the home was a mess, with articles of trash and clothing,animal feces and other
debris strewn about. The bathroom had bare studs in the shower area an the smoke detectors throughout the
dwelling were hanging down with batteries removed. The rear egress was obstructed with trash and debris
making it unusable.The cellar bulkhead was not able to be secured which left the home open to the elements.
Under the authority of the Mass State Building Code 780 CMR sections 116.1 through 116.3 1 declared the
house dangerous and unsafe. I also am removing the occupancy permit for this property. It is unlawful to
occupy this structure until such time as the code deficiencies are corrected and inspections are conducted.
If you feel you are aggrieved by this order, your appeal is to the Board of Buildi8ngs , Regulations and
Standards in Boston.
I have taken the action of re-keying the property and having the rear bulkhead boarded up to secure the
property. I also had a pile of rugs and foam rubber removed from the front of the property.

The ensuing communication shall serve as our follow-up to our April 19,2012; Required Inspection letter
sent to Mr. Charles DiMino under the provisions of 780 CMR, Section 104.6—Right of Entry, of the State
Building Code. The inspection was conducted on Thursday, April 26,present were Salem Police Officers
Mr. Robert Phelen and Mr. Harry Rocheville, Mr. David Greenbaum and Ms. Delilah Castro of the Salem
Health Department, Mr. Charles DiMinio your tenant and myself. The purpose of the investigation was to
review complaints noted in April 7, 2012 Police and Fire Department report's forwarded to our
department and refered to in the aforementioned letter of April 19, 2012.
Let it be noted that violations stated in this correspondence are solely matters that are governed by this
office and the Massachusetts State Building Code 780 CMR,but are not the only items of concern on the
property. Violations regarding smoke detectors, carbon monoxide detectors, egress doors and
hallways shall be corrected with-in twenty four hours of receipt of this letter,this department
thanks you for your anticipated direct attention to that matter. The following items remain
outstanding and/or were noted during our visit of April 26`s
Therefore, under the authority of the Mass State building code 780 C.M.R, you are directed to address the following:
1- Smoke detectors shall be installed outside each bedroom location and at the top and bottom of all
stairways. PARTIALLY CORRECTED AS OF APRIL 26, 2012 at 6:05 p.m.,ADDITIONAL
SMOKES NEED TO BE ADDED AT STAIRWAYS AND ONE AT FIRST FLOOR
BEDROOM AA CROSS FROM BATHROOM.
2- Carbon Monoxide detectors shall be installed outside of bedroom areas within ten feet of all
bedrooms and one installed in the basement level. CORRECTED AS OF APRIL 26, 2012 at
6:05 p.m.

3- First floor room off of hall adjacent to basement stair at front of house is filled with trash bags,
papers, trash bags,furniture and other unidentified items all piled on top of each other. The room
could not be accessed for full inspection, as the trash measured seven feet in height. THIS ROOM
MUST BE CLEARED IMMEDIATIE Y AS STATED TO MR. DIMINO AT TIME OF THE

4- Side entry door off kitchenlporch area is inoperable and needs to be repaired immediately as
presently the dwelling has no secondary egress. CORRECTED AS OF APRIL 26, 2012 at
6:05 p.m.
5- Bathroom tub/shower unit needs to be replaced; wall framing is failing, mold, and plastic
covering not an acceptable correction method of correction.
6- Basement level room at bottom of stairs must immediately cease and desist being used as a
bedroom, as it does not meet life safety and egress requirements. CORRECTED AS OF APRIL
26, 2012 at 6:05 p.m.
7- Fire code gypsum wall board is missing from stud wall adjacent to furnace,presently fiberglass
insulation is exposed.
8- Basement room at bottom of basement stair at front of house is filled with trash bags,papers,
trash bags,furniture and other unidentified items all piled on top of each other. The room could
not be accessed for full inspection, as the trash measured seven feet in height. THIS ROOM
MUST BE CLEARED IMMEDIATLEYAS STATED TO MR. DIMINO AT TIME OF THE
INSPECTION,INCLUDING DOCUMENTATION OF WHAT COMPANY IS HAULING

Our office received a complaint regarding life safety and sanitary conditions at your property located at
22 Charles Street,these complaints have also been supported by concerns expressed by Salem Prevention
officials.
The most concerning off these violations that has been received are the life safety, egress and sanitary conditions at the
property, Required Inspection must be conducted by our Department to assure compliance with the State Building code
and city ordinances. It is requested that all egress stairways and exit doors be cleared immediately of any and all
storage, debris and other items.
Under the provisions of 780 CMR, Section 104.6—Right of Entry, of the State Building Code, access to this property
must be granted for the purposes of this inspection. Please call this office upon receipt of this letter, ifthis property has
rental units, these tenants must be notified in advance of this inspection, so that access to these spaces may also be
accomplished.
This Required Inspection shall be conducted by this office on Thursday,April 26, 2012 at 1:30 p.m.; failure to respond
to this notification will be construed as non-compliance,with issuance of Municipal tickets and as such an
