1. Basement has numerous violations. It appears this property is being
used as a two-family occupancy without proper exits out of basement apartirent:'

original installation. Apparent lack of approved controls on burner.

3. No further repairs to this unit will be

with provisions of FPR #3. Automatic damper on flue pipe shows evidence
of having been locked in open position. Heavy coating of soot shows
{' that gases and fumes never reached chimney and ware vented in basement. * '`
Chimney cleanout had broken bricks and debris inside.
Building Inspector will have to evaluate this condition.
4. It appears wiring in basement does not meet code requirements. Further
investigation by Electrical Inspector required.
. S. 'Gas installation of domestic hot water heating unit appears improper
and exhaust of this unit seems to empty inside building. Gas Inspector to
