construction specifications, and in compliance and in accordance to the design provided by
Gienapp Design Associates. HOME IMPROVEMENT REQUIREMENTS: H.I.C. #100273. The owner is
allowed the right to cancel for three business days under MGL c. 93, s. 48, MGL c. 140d, s.
10 or MGL c. 255d s. 14, as may be applicable. All warranties, and the owner's rights come

CONTRACTORS RESPONSIBILITIES
This outline is intended to establish minimum quality standards for build out of
16'4"x 20'-0"bedroom addition,minor interior renovations and new bulkhead on 12
Orleans St.,but is not intended as a complete specification for construction purposes. The
renovations shall be constructed to the design provided by Gienapp Design Associates,
LLC (specifically Scheme 2 (dated 12.21.06)without the bathroom). It is understood that the
contractor will make all necessary provisions to ensure that the renovation will be in
compliance with all applicable local, state codes, and regulations. It is further understood
that construction will be completed in a workmanlike manner in keeping with the
standards of the industry.
1. DEMOLITION
• All demolition necessary for the construction renovations in accordance to plans.
• Demolition of existing concrete clothes line pad.
• Provide on site containers if possible or removal by truck of all job related debris.
2. EXCAVATION/FOUNDATION/SLAB
• All excavation necessary for the installation of foundation in accordance to plans
Note:No ledge work removal is included in this price. If ledge is encountered
MDB will pin to the ledge.
• Foundation to consist of all necessary footings,and foundation walls in accordance tq/-iP
supplied plans, and above note.
• Completion of excavation,backfilling to be rough grade only. ��t
• Installation of bituminous foundation coating on all new foundation walls.
Note: No perimeter drain work has been included in this proposal for the existing
home, and may be necessary to insure a dry basement. Due to ledge this item is probably
impossible.
• Installation of 6 mil vapor barrier and'/4" crushed stone in crawl space area.
3. PARTITIONS
EXTERIOR:
All exterior partitions shall be constructed of 2x4 wood stud, 16"o.c., '/2"
plywood sheathing(CDX), Tyvek air infiltration barrier, White Cedar shingles to match
existing as close as possible, 3 '/2",(R-13)wall insulation, '/2 blueboard(skim coat
plaster).
INTERIOR:
All new interior partitions shall be 2x4 construction, 16"o.c., '/2, blue board
(plastered).

Wood joist floor framing to be 2x10 hem/fir, V plywood subfloor(CDX), %2 AC, or
Equal finish plywood attached as necessary,R-19 insulation.
• Finish floor to be supplied and installed by.others.
• MDB to reuse existing hardwood flooring in order to patch floors at old wall
locations,but is not responsible for refinishing.
5. ROOF/CEILING
• Wood frame system to be constructed of 2x10 wood rafters,2x8 ceiling joists, '/2"
blueboard(skim coat plaster(textured)), R-30 insulation(new only), 5/8"plywood,
15 LB felt paper, standard asphalt roof shingles, lx6#2 rakes, lx8 #2 facias, 3/8"
plywood soffit, ,and ridge vent system.
6. WINDOWS/DOORS
• Andersen windows to have a materials allowance of$1,800.00
• All new interior doors to be hollow core luan pre-hung door units as per plans.
• Exterior door to be Therma-tru, or equal steel door units, with no storm door and to
be included in above allowance.
• New steel bulkhead door unit to be Bilco or equal standard size unit.
7. PLUMBING
• No plumbing has been included and should not be necessary.
8. HEATING
• Installation of new supply and return for new bedroom.
Note: No upgrading of boiler has been included in the cost of this project.

10. SPECIALTY ITEMS
• Installation of all necessary wood trim to match existing as close as possible.
• Installation of egress windows in existing bedroom
• Installation of existing wall changes as per plans.
• Installation of two shelves and clothes poles in each new walk in closet, one shelf and
clothes pole in new existing bedroom closets and 5 shelves in new linen closet.
• MDB to supply all building permits,owner to supply any variances or special permits
if necessary.
• Owner to supply plot plan as required by building dept.
11. EXCLUSIONS
• Any unforeseen conditions.
Note: MDB describes an unforeseen condition as any additional work required
due to a condition that was not visible or known by the builder during the walk through or
pricing stage.
• Repairs to any landscaping,or driveways caused by dumpsters,trucks, excavation,
etc.
Note: MDB will do their best to protect property,but cannot be held responsible
for normal construction damage.
• If the homeowner chooses to reside in the home during renovations, MDB
Construction will not be responsible for problems due to airborne particulate,
hazardous construction situations or homeowner caused delays.
• It is the intent of the contractor to have a working crew Monday through Friday 7:30
to 4:00. It will be the homeowner's responsibility to ensure that the job site is in a
work ready environment during our crews working hours.

• MDB Construction will not be responsible for moving or protection of homeowner's
furniture and personal possessions due to homeowner residing in the home during the
renovations.
• MDB to not be responsible for any finished floors,painting or staining.
• MDB Construction to not be responsible for any variances, and or special permits.
12. ADDITIONAL WORK
The Contractor shall perform additional work as requested and approved,in writing, by
The Owner and the Contractor for a sum specified. The Contractor shall make at any
time during the life of this Agreement any revisions,additions,deletions,modifications,
corrections, substitutions or changes to the Scope of Services as may be agreed to by both
parties to this Agreement for a sum specified. .
13. INSURANCE
The Contractor warrants that it is fully covered by the types of insurance listed below and
that it will maintain said insurance at all time while the Work is being performed. When
requested, the Owner shall be provided with Certificates of Insurance.
1. Workers Compensation as required by the law of Massachusetts;
2. Comprehensive General Liability Insurance for a combined single limit of one
Million dollars($1,000.000)for bodily injury, death, and property damage;
NOTE: It is the intent of the contractor to build to the design provided. The detailing in
this specifications sheet is meant as a guideline, and not meant to be exhaustive
14. MISCELLANOUS
• Please take the time to read your contract carefully. All owners or parties responsible
for payment must sign the contract where noted.
• Payments are considered due on receipt, and will incur finance charges, and late fees.
• This contract will expire 6 months after contract signing if project is not started due to
zoning and or homeowner delays.
PAYMENT SCHEDULE
1) $ 1,500.00 upon signing(to be used to procure permit)
2) $15,000.00 upon project start.
3) $10,000.00 upon foundation start.
4) $10,000.00 upon framing start.
5) $10,000.00 upon window, and siding start.
6) $10,000.00 upon electrical rough inspection by building dept.
7) $10,000.00 upon start of blueboard, and plaster.
8) $ 5,000.00 upon start of interior doors, and wood trim.
9) $ 3,000.00 upon substantial completion
10) $ 2,500.00 upon completion.
