In accordance with provisions of Section II-B, the
applicant must file, by delivery or registered mail, a t
Notice with the City Clerk stating the date of sub-
mission for such determination. The notice shall be
attached to two copies of this Form A application.
The notice and both copies of the application must
be "date stamped" by the City Clerk and then one
copy of this Form A, with the Plan, filed with the
Planning Board by the applicant. All notices and
applications shall be typewritten or neatly printed
in ink.

This request only involves the changing of existing lot
line to conform with Sub-division Plan Lot 304 , Cert . Title
# 3862 , South Registry District of Essex County .
To Planning Board Members :
I request this Form "A" Approval for Mr. Dominic Ingemi
at this time so that the portion of Lot 304 , titled Lot A, can
become legally conveyable along with Lots 301 and 302 .
Due to a misconception at the time of original sale , Mr.
Ingemi purchased 301 and 302 and Lot "A" of 304 from his
parents . This took place in May , 1943 . It was assumed at that
time that 304 A & B was unregistered land. Therefore, only Lots
301 and 302 were registered in his name. Only recently has
it come to light that Lot 304 was registered.
This brings us to this point this evening. In order
for Mr. Ingemi to assume legal registered title to Lot 304 "A",
he has had prepared a new plan encorporating Lot 304 "A" into
one continuous parcel along with Lot 301 and 302. This plan
conforms to all Massachusetts Land Court standards recognized
as controlling this request.
This plan must be endorsed by this board since it will
be showing a division of "Old Lot 304."
