(978) 741-1800 KIMBERLEY DRISCOLL health@salem.com DAVID GREENBAUM,RS MAYOR HEALTH AGENT December 6, 2022 Jon-Heath Realty Trust Attn: Heather Klosowski 30 Bridge Street South Hamilton, MA 01982 Dear Madam: In accordance with Chapter II of the State Sanitary Code, 105 CMR 410.000, Minimum Standards of Fitness for Human Habitation, a recent reinspection was conducted of your property located at 11 Summer Street Unit 1 L, conducted by Jeffrey Barosy, Sanitarian of the Salem Board of Health on December 6, 2022. At that time the following items were still outstanding: Item#1: The living room ceiling still shows signs of water leakage with brown discoloration of paint. Item#2: The bathroom ceiling above the bathtub still has cracking and peeling paint. Item #3: The bathroom wall where it meets the bathtub above the wall mounted radiator is still deteriorating. Item#4: Occupant stated she is still seeing mice. Occupant informed inspector an exterminator visited the property a month prior to the re-inspection. Please let the inspector know what work was done and what work is to be done in the future to eliminate the presence of mice in the unit. Item #5: Rear porch still has chipping and peeling paint on its overhead wooden boards. Item#6: One portion of the rear porch's floorboards have been repaired but there is still another section that is covered with a wooden plank, behind one of the porch walls facing the parking lot. Item#8: Refer to Lead Determination documentation that was mailed separately. Please correct the outstanding violations and contact this office to schedule a final re-inspection of this apartment. All other violations noted in the inspection report of October 6, 2022 have been corrected.
